REGISTRATION CHECKLIST: 

Registration at any CCSS school requires four proofs of residence.

MUST HAVE THE FOLLOWING: 

1. Evidence of ownership or lease of an inhabitable residence located within the Central School District.

MUST HAVE THREE OF THE FOLLOWING:

A. Evidence of utilities, ONE of the following for the last TWO months: (water, gas, electricity, home telephone, or cable) at the residence located within the Central Community School District.

B. Property tax bill or homestead exemption application for preceding tax year.

C. Current medical or social security insurance addressed to residence located within the Central School District.

D. Current Louisiana Driver’s License or voter registration card at the residence located within the Central School District.

E. Automobile insurance for the previous (2) months or homeowners/rental insurance addressed to resident in the Central School District.

F. Income Tax Return for the most recent filing period, listed with the residence located within the Central School District.

THE FOLLOWING DOCUMENTS ARE ALSO REQUIRED FOR EACH STUDENT:

Valid Birth Certificate

Social Security Card

Current Shot Records

Most Recent Report Card & Standardized Test Scores

Drop Slip from Previous School

Official Signed Custody Papers if Applicable

Louisiana Residency Form

Course Scheduling Form

*Please note that all homeschool students must take a placement test to determine grade level.
